Home Health RN Case Manager
Job DescriptionAmcare Pro Home Health is a mid-size home health care company with offices in East Texas and Dallas.
The Dallas office is a stable operation & census; including a well-seasoned office and field staff.
We are currently interviewing for a Full Time Home Health RN Case Manager position to cover Garland and its surrounding areas.
The position will involve home care visits for Admits, Recerts, Sup & SN visits.
Previous Home Health Care experience is a must.
Familiarity with electronic charting (WELLSKY/KINSER) is a plus.
Job DetailsThe RN position will be required to perform home health SN Initial Assessments, OASIS Admissions, Re-cert visits, ensure compliance with all State regulations.
The RN must have knowledge/experience with home health forms, OASIS charting, Medicare guidelines, rules & regulations.
Qualifications Must currently be licensed in the state of Texas Should have at least 1 year nursing experience IV infusion experience is a MUST Prior home health care experience is a MUST Should have Reliable Transportation & Car insurance Must be able to pass background check & have valid CPR Excellent compensation with benefits If interested, please respond to this posting with a current resume and cover letter for further consideration.
