Job Description:
This is a Night Position- Shift 7pm-7am on a rotating schedule! This is an onsite position, located in The Colony, Texas.
However, there is no direct patient care.
We are offering a $4,000.
00 retention bonus.
There is an additional $1.
25/hour shift differential for Weeknight and an additional $3.
00/hour for Weekend shifts.

JOB SUMMARY This position will be responsible for linking mobile resources together, integrate and coordinate them to respond appropriately to patient needs.
Telephonic patient triage following established evidence-based protocols to assist in navigating care across the health care continuum.
Responsible for telemonitoring and patient education activities.
Actively coordinates team care in a virtual setting.
Additional duties include leadership roles and mentoring as assigned.
ESSENTIAL FUNCTIONS/DUTIES o Demonstrates the ability to function as a Registered Nurseo Telephonically triages patients and navigates care based upon evidence-based protocols.
o Patient education across the health care continuum.
o Telemonitoring activities engaging healthy patient behaviors.
o Must demonstrate effective communication skills and be able to work well with others including team members, patients, caregivers, referral sources, physicians and their staff, facility staff members and the community.
o Demonstrates ability to effectively learn and use computer and required software programso Participate in activities that promote the GMR Integrated Healthcare Program and the GMR organization at large.
o Must be able to act as a facilitator and educator for the Integrated Healthcare training course provided by GMR.
o Must demonstrate the ability to manage time, resources and paperwork requirementso Provide excellent customer serviceo Perform other duties as assignedQUALIFICATIONS
Experience:
o 5 years or more of RN experience in Emergency or Acute Care settingEducation:
o Graduate of an accredited school of vocational nursingo Master's or Bachelor's degree in Healthcare Administration, Public Health or any other degree with a focus on community or disease education;Skills:
o Strong and effective verbal, written, and interpersonal communication skills.
o Nursing skills as defined as generally accepted standards of practice.
Good interpersonal skills.
o Ability to adhere to established standards for educational quality.
o Familiar with basic computer applications and functions.
o Proficient Bilingual translations skills (prefer)o Demonstrated teaching and educational facilitation skills.
Credentials/Licenses:
o Maintain current RN state Nurse license with ability to obtain multi state licensureo Current CPR card.
