Position
Summary:
The Intake Nurse is primarily responsible for ensuring that intake information is accurate and complete.
Responsible for entering patient demographics into computer database according to company policy and assisting in setting up any and all home services required for new referrals.
Education/
Experience:
Graduate of an accredited school of nursing, BSN preferred.
Current state license as Licensed Vocational Nurse (LVN), Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing.
Continuing education necessary to maintain current licensure in all states licensed.
Ability to multi-task, Excellent customer service skills, Excellent computer and typing skills necessary for company workflow and completion of daily tasks.
Minimum of two years current nursing experience in a clinical specialty involving infusion therapy, including patient assessment skills, venous access skills, and emergency patient care.
Minimum of one year previous specialized home health experience desired.
Bilingual preferred.

Director, Home Infusion Nursing Operations for policy matters relating the home infusion intake process and clinical oversight.
